Tristan’s Case: When Strength Doesn’t Protect You From Sciatica
Tristan is not your typical patient.
He’s a bodybuilder and gym owner , someone who understands training, movement, and physical discipline at a high level. He was referred into our office by another gym owner who had already experienced care in our clinic.
Despite his strength and conditioning, Tristan came in with sciatica and persistent low back pain.
That’s something many people don’t expect, that even highly trained individuals can still develop structural issues that affect the spine and nervous system.
Because when the issue is structural, strength alone doesn’t fix it.
What His X-Rays Revealed
With Gonstead chiropractic, we don’t rely on guesswork. While many practitioners can feel that something is off, we use full-spine X-rays (when appropriate) to confirm exactly what’s happening.
Tristan’s case revealed several important findings:
- Six lumbar vertebrae instead of the typical five
- Spondylolisthesis at L6
- Partial fusion of L6 to the sacrum
This means part of the vertebra had either developed differently or experienced structural damage over time. In some cases, this can be congenital. In others, it may relate to stress or trauma.
Interestingly, many people with spondylolisthesis don’t even know they have it. Others can trace it back to activities involving repeated spinal extension — like gymnastics or certain types of training.
But when it becomes symptomatic, that’s when it matters.
How This Case Was Managed
Even with complex findings, the approach remains focused and specific.
In Tristan’s case, analysis pointed to the sacrum as the primary area of concern. Instrumentation confirmed abnormal readings at the sacrum, and X-rays showed posterior positioning.
The adjustment was focused there.
Post-adjustment instrumentation showed a clear change, indicating improved nerve function in that region.
This is a key principle of Gonstead chiropractic: you don’t chase symptoms, you correct the primary area of interference.
Understanding Sciatica, Spondylolisthesis, and Nerve Interference
A subluxation occurs when a spinal segment loses proper position and function, creating interference in the nervous system.
Because the nervous system controls how the body functions, that interference can show up in many ways:
- Sciatica and radiating leg pain
- Chronic low back pain
- Muscle tightness or spasm
- Fatigue and decreased performance
- Pain with standing, lifting, or training
Muscle spasm, for example, is often not the primary problem, it’s a response to nerve irritation.
What Changed
After care, Tristan reported improvement in:
- Sciatica
- Low back pain
- Fatigue
- Glute tightness
- Pain with prolonged standing
Even in complex cases, when the right area is addressed, the body can begin to function differently.

Questions About Complex Sciatica Cases
Can you have sciatica with structural abnormalities?
Yes. Conditions like spondylolisthesis or spinal variations can contribute to nerve irritation depending on how the spine is functioning.
Is spondylolisthesis always painful?
No. Many people have it and never know. It becomes relevant when it starts affecting nerve function or stability.
Why use X-rays in chiropractic?
X-rays help confirm structure, alignment, and anomalies so care can be more precise rather than guess-based.
Can athletes still have spinal problems?
Absolutely. Strength and conditioning don’t eliminate structural issues in the spine.
Do all sciatica cases need surgery?
No. Many cases can be managed conservatively depending on the situation. It’s always reasonable to explore options and get multiple opinions.
What makes Gonstead different in complex cases?
It focuses on specificity — identifying the exact level of dysfunction and correcting it precisely.
Don’t Assume Your Case Is Too Complicated
Tristan’s case had multiple structural findings — and still improved.
If you are dealing with sciatica, low back pain, or have been told you have a complex spinal issue, the next step is understanding what is actually happening.
